REQUEST FOR RELEASE OF FUNDS

On or about June 13, 2017 the City of Urbana will submit a request to the Dept. of Housing & Urban Development (HUD) for the release of Community Development Block Grant (CDBG) funds under Title I of the Housing and Community Development Act of 1974 (PL93-383), as amended, and HOME Investment Partnerships Program (HOME) funds under Title II of the Cranston-Gonzalez National Affordable Housing Act, as amended, to undertake the following projects:

CDBG: Housing Activities, Planning and Administration, Public Facilities, Public Services

HOME: Champaign County Neighborhood Revitalization, City of Champaign Neighborhood Revitalization, CHDO Operating, CHDO Project, City of Urbana Neighborhood Revitalization, Planning and Administration, Tenant-Based Rental Assistance

FINDING OF NO SIGNIFICANT IMPACT

The City of Urbana has determined that the project will have no significant impact on the human environment. Therefore, an Environmental Impact Statement under the National Environmental Policy Act of 1969 (NEPA) is not required. Additional project information is contained in the Environmental Review Record (ERR) on file at the City of Urbana, 400 South Vine Street, Urbana, IL 61801 and may be examined or copied weekdays 8:00 A.M to 5:00 P.M.

PUBLIC COMMENTS

Any individual, group, or agency may submit written comments on the ERR to the City of Urbana Grants Management Division. All comments received by June 12, 2017 will be considered by the City of Urbana prior to authorizing submission of a request for release of funds. Comments should specify which Notice they are addressing.

ENVIRONMENTAL CERTIFICATION

The City of Urbana certifies to HUD that Diane Wolf Marlin in her capacity as Mayor of the City of Urbana consents to accept the jurisdiction of the Federal Courts if an action is brought to enforce responsibilities in relation to the environmental review process and that these responsibilities have been satisfied. HUD’s approval of the certification satisfies its responsibilities under NEPA and related laws and authorities and allows the City of Urbana to use Program funds.

OBJECTIONS TO RELEASE OF FUNDS

HUD will accept objections to its release of fund and the City of Urbana’s certification for a period of fifteen days following the anticipated submission date or its actual receipt of the request (whichever is later) only if they are on one of the following bases: (a) the certification was not executed by the Certifying Officer of the City of Urbana; (b) the City of Urbana has omitted a step or failed to make a decision or finding required by HUD regulations at 24 CFR part 58; (c) the grant recipient or other participants in the development process have committed funds, incurred costs or undertaken activities not authorized by 24 CFR Part 58 before approval of a release of funds by HUD; or (d) another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental quality. Objections must be prepared and submitted in accordance with the required procedures (24 CFR Part 58, Sec. 58.76) and shall be addressed to the Dept. of Housing and Urban Development Region V Office, Attn: Environmental Office, 77 West Jackson Blvd., Chicago, IL 60604-3507. Potential objectors should contact HUD to verify the actual last day of the objection period.

The quantities appearing in the bid schedule are approximate and are prepared for the comparison of bids. Payment to the Contractor will be made only for the actual quantities of work performed and accepted or materials furnished according to the contract. The scheduled quantities of work to be done and materials to be furnished may be increased, decreased or omitted as hereinafter provided.

Submission of a bid shall be conclusive assurance and warranty the bidder has examined the plans and understands all requirements for the performance of work. The bidder will be responsible for all errors in the proposal resulting from failure or neglect to conduct an in depth examination. The Awarding Authority will, in no case be responsible for any costs, expenses, losses or changes in anticipated profits resulting from such failure or neglect of the bidder.

The bidder shall take no advantage of any error or omission in the proposal and advertised contract.

If a special envelope is supplied by the Awarding Authority, each proposal should be submitted in that envelope furnished by the Awarding Agency and the blank spaces on the envelope shall be filled in correctly to clearly indicate its contents. When an envelope other than the special one furnished by the Awarding Authority is used, it shall be marked to clearly indicate its contents. When sent by mail, the sealed proposal shall be addressed to the Awarding Authority at the address and in care of the official in whose office the bids are to be received. Of a Data Security Incident

This Notice of a Data Security Incident (“Notice”) is intended to inform potentially affected individuals of a data security incident and to provide them with information that they may if they elect to take additional actions as a result of the incident. The Federal Savings Bank (“TFSB” or the “Bank”) recently became aware of a potential data security incident involving certain Illinois residents who are not TFSB account holders but whose names and Social Security numbers are potentially at risk as a result of the theft of a Bank employee’s laptop computer. We want to emphasize that the incident does not involve unauthorized access to our data systems and the document described in this Notice does not contain the personal information of any Bank account holder.

Recently, a TFSB employee’s laptop was stolen from the employee’s personal vehicle while the vehicle was not on Bank property. The laptop is protected by a strong password and the employee promptly reported the theft to the Bank and the Chicago Police Department. The Bank is cooperating in the police investigation. Following the theft, the Bank undertook a thorough investigation which revealed that the laptop contained a document listing Illinois residents (first name, last name, and associated Social Security number) who owed past-due child support payments as of July 8, 2016. The document did not include any other information.

The Bank has no information to-date indicating the misuse of any of this personal information.

The Bank, and other banks that operate in Illinois, received the document in question in order to fulfill a state law mandate (305 Ill. Comp. Stat. § 5/10-24.5) that requires banks to compare data concerning account holders with individuals identified by the Illinois Department of Healthcare and Family Services as owing past-due child support.

Potentially affected individuals who have questions may contact the Bank’s call center from 9:00 AM to 5:00 PM (central time), Monday through Friday, at 844-489-3572.
