1.04 TO: POTENTIAL BIDDERS
A. Your firm is invited to submit an offer under seal to Owner for renovation of efficiency apartment units located at 723 North Oak St, Danville, IL 61832. Bids are due at the office of the Danville Housing Authority, 1607 Clyman Ln., Danville, IL before 2:00 pm local standard time on the 9th day of October, 2014, after which bids will be opened and read aloud publicly. Bids must be clearly marked as"BID - DHA Mer Che Apartment Renovations.
B. Project Description: The Danville Housing Authority requests bids for general contractor work that will be done as part of the renovation at MER CHE HOUSING located at 723 North Oak St, Danville, IL 61832. Work includes, but is NOT LIMITED TO: interior remodeling of a total of 11 (eleven) selected existing efficiency apartment units. Existing apartment building is seven floors; renovation work will include work on individual units on floors 3, 4, 5, 6, & 7. Interior work for these units will include removal of existing interior wall framing, existing floor and wall patching, new interior walls, new floor and ceiling finishes, new cabinets and countertops, new doors, frames and hardware, and new restroom fixtures and accessories. Work will also include renovations to plumbing, heating, and electrical systems within the units.
C. Bid Documents for a Stipulated Sum contract may be obtained from the office of the Architect upon receipt of a refundable deposit, by cash or check, in the amount of $100 for one set. Contractors may be mailed sets for a non-refundable $15 fee. Electronic sets may be obtained free of charge. No partial sets will be distributed.
D. A Pre-bid meeting will be held at the site on Tuesday September 30, 2014 at 10:00am.
E. Bidders will be required to provide Bid security in the form of a Bid Bond of a sum no less than 5 percent of the Bid Amount. A 100% Performance/Payment Bond will be required.
F. Contractors will be required to comply with all laws, including those relating to minimum wages, overtime pay requirements for employees, nondiscrimination/equal opportunity rules (Executive Order 11246), "Section 3" of the Housing and Urban Development Act of 1968, and prevailing wage rates of the various classes of work to be performed under the proposed contract, in accordance with Federal Labor Standards provisions, US Dept of Labor.
G. Refer to other bidding requirements described in Document HUD 5369 -Instructions to Bidders for Contracts Public & Indian Housing Programs.
H. Your offer will be required to be submitted under a condition of irrevocability for a period of sixty (60) days after submission.

NOTICE TO HUSBAND
Brian Matthew Teague, husband of Heather Nicole Lee-Teague, who delivered a child on August 31, 2014, is notified that a Petition for Adoption of the child was filed in the Office of the Clerk of the Marion County Superior Court, Probate Division, City-County Building, Indianapolis, Indiana. Heather Nicole Lee-Teague has said that you are not the father of the child.
If Brian Matthew Teague seeks to contest the adoption of the child, he must file a Motion to Contest the Adoption in accordance with I.C. 31-19-10-1 in the above-named Court within thirty (30) days after the date of service of this Notice. This Notice may be served by publication. If Brian Matthew Teague does not file a Motion to Contest the Adoption within thirty (30) days after service of this Notice, Brian Matthew Teague’s Consent will be irrevocably implied and Brian Matthew Teague loses the right to contest the adoption or the validity of his implied consent to the adoption.
Nothing Heather Nicole Lee-Teague or anyone else says to Brian Matthew Teague relieves him of his obligations under this notice.
Brian Matthew Teague is being given this Notice because, under Indiana Law, he is presumed to be the father of the child because the child was born during his marriage to Heather Nicole Lee-Teague.
This notice complies with I.C. 31-19-4.5-3, but does not exhaustively set forth a person’s legal obligations under the Indiana adoption statutes. A person being served with this notice should consult the Indiana adoption statutes.
