Champaign CUSD No. 4 is requesting two separate not-to-exceed cost proposals to provide material testing services for Central High School Additions & Renovations and Centennial High School Additions & Renovations. These projects consists of building demolition, excavation, backfill, concrete, steel, masonry, and other various construction activities at both schools.

The construction schedule managed by O'Shea Builders on behalf Champaign CUSD No. 4 is as shown in the available exhibits, including the following project milestones:

Contract Award/Notice to Proceed: Centennial: March 25, 2019, Central: April 22, 2019

Substantial Completion: Centennial: July 31, 2021, Central: July 31, 2022

Final Completion: Centennial: August 31, 2021, Central: August 31, 2022

Sealed bids, in duplicate, will be received by Champaign Community Unit School District #4 up until 2:00PM (CDT) on Monday April 1, 2019.

Scope of Material Testing Services

Your proposal should include the following scope of work. If more or less testing and inspection is required than that specified below for adequate evaluation, such recommendations shall be made to the Owner before proceeding.

• Perform field testing, inspections, provide testing reports, and make professional recommendations in accordance with Divisions 03-Concrete, 05-Metals, Division 31-Earthwork, Division 32-Exterior Improvements.

All testing shall be according to ASTM Standards. Your proposal shall provide unit prices for each item of testing and inspection and the actual final cost will be based on the actual units used. Adequate provisions shall be made such that the resulting total cost of testing and inspection shall represent a "maximum not to exceed" cost. Outline standard notices required to schedule testing and normal working hours for services. State all exclusions from your scope. Include a standard hourly fee schedule for field testing services and engineering services as well as a unit cost schedule of laboratory testing services.

Site visits shall be determined by progress of construction, followed with written documentation of observed construction deficiencies and reporting such to the Owner, Architect and General Contractor.
