NOTICE IS HEREBY GIVEN that the Plan Commission of the City of Urbana, Illinois will hold a public hearing on Thursday, March 7, 2019 at 7:00 P.M. in the Urbana City Council Chambers, 400 South Vine Street, Urbana, Illinois, at which time and place the Commission will consider the following applications:

ZONING ORDINANCE TEXT AMENDMENT
NOTICE IS HEREBY GIVEN that the Plan Commission of the City of Urbana, Illinois will hold a public hearing on Thursday, March 7, 2019 at 7:00 p.m. in the Urbana City Council Chambers, 400 South Vine Street, Urbana, Illinois, at which time and place the Commission will consider the following rezoning:
Plan Case 2372-T-19: An application by the Urbana Zoning Administrator to amend Article II (Definitions), and Article V (Use Regulations) and Table V-1 of the Urbana Zoning Ordinance to clarify the regulations of day care home occupations and allow Day Care Facilities to be a permitted use in all business districts.
The Urbana Zoning Administrator is proposing to amend the Zoning Ordinance to amend Article II (Definitions) to add and clarify definitions; amend Article V (Use Regulations) to align better with existing state regulations for day care homes; amend Article V (Use Regulations) Table of Uses (Table V-1) to permit Day Care Facilities in all business districts by right, and to make other related changes as deemed necessary.

Village of Fisher – 3rd Street Resurfacing
Sealed proposals for the improvements described below will be received at the office of the Fisher Community Center, 100 E. School Street, Fisher, IL until 1:59 PM on March 5, 2019.
Sealed proposals will be opened and read publicly at the office of the Fisher Community Center, 100 E. School Street, Fisher, IL at 2:00 PM on March 5, 2019.
This section includes HMA resurfacing of 3rd Street from Fairfield Street to the north corporate limits.
Plans and proposal forms will be available in the office of Fehr Graham at 1610 Broadmoor Drive, Champaign, IL 61821. Bid documents may be obtained from the Engineer electronically by emailing tovermyer@fehr-graham.com.
Prequalification is required. The 2 low bidders must file within 24 hours after the letting an “Affidavit of Availability” (Form BC 57), in duplicate, showing all uncompleted contracts awarded to them and all low bids pending award for Federal, State, County, Municipal and private work. One original shall be filed with the Awarding Authority and one original with the IDOT District Office.
The Awarding Authority reserves the right to waive technicalities and to reject any or all proposals as provided in BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals.
The following BLR Forms shall be returned by the bidder to the Awarding Authority:
a. BLR 12200: Local Public Agency Formal Contract Proposal

The quantities appearing in the bid schedule are approximate and are prepared for the comparison of bids. Payment to the Contractor will be made only for the actual quantities of work performed and accepted or materials furnished according to the contract. The scheduled quantities of work to be done and materials to be furnished may be increased, decreased or omitted as hereinafter provided.
Submission of a bid shall be conclusive assurance and warranty the bidder has examined the plans and understands all requirements for the performance of work. The bidder will be responsible for all errors in the proposal resulting from failure or neglect to conduct an in depth examination. The Awarding Authority will, in no case be responsible for any costs, expenses, losses or changes in anticipated profits resulting from such failure or neglect of the bidder.
The bidder shall take no advantage of any error or omission in the proposal and advertised contract.
If a special envelope is supplied by the Awarding Authority, each proposal should be submitted in that envelope furnished by the Awarding Agency and the blank spaces on the envelope shall be filled in correctly to clearly indicate its contents. When an envelope other than the special one furnished by the Awarding Authority is used, it shall be marked to clearly indicate its contents. When sent by mail, the sealed proposal shall be addressed to the Awarding Authority at the address and in care of the official in whose office the bids are to be received. All proposals shall be filed prior to the time and at the place specified in the Notice to Bidders. Proposals received after the time specified will be returned to the bidder unopened.
Permission will be given to a bidder to withdraw a proposal if the bidder makes the request in writing or in person before the time for opening proposals.

The selection committee has set the following selection criteria and weighting: Specialized Expertise with Natural Resources 20%, Staff Capabilities 20%, Past Performance 20%, Local Presence 10%, Experience with Public Funding 10%, Technical Approach 10%, and Work Load Capacity 10%. The Statement of Qualifications must not contain any cost information. All submittals shall include a conflict of interest form. Interviews may be conducted with one or more responsible entities that have submitted proposals in order to clarify certain elements, if such information cannot be satisfactorily obtained over the telephone or via e-mail.
The project is being funded, in part, with Illinois Transportation Enhancement Program (ITEP) funds made available through the Illinois Department of Transportation. ITEP is a federal funding source.
Engineering firms will be required to comply with all federal, state and municipal laws and regulations, including those related to discrimination, prevailing wage, safety, drug-free workplace, bid rigging, human rights, preferential employment of Illinois labor and equal opportunity employment. Minority and female-owned businesses are encouraged to submit proposals.
