State disciplines licensees

SPRINGFIELD — The Illinois Department of Financial and Professional Regulation has announced the following disciplinary orders for June:

— The physician and surgeon license and the controlled substance license of Kurt Dearnbarger of Sullivan was indefinitely suspended for abusing alcohol and illegal drugs.

— The real estate broker license of Curtis Shoot of Champaign was placed on probation for two years for having a felony conviction from 2011 for theft.

— U.S. Cash Advance Illinois LLC, of Danville, was fined $5,000 for the following violations: Licensee did not maintain a positive net worth of a minimum of $30,000.

The annual percentage rate is not accurately disclosed. The licensee did not mail or deliver to the borrower a monthly billing statement describing the conditions under which the charge for interest may be made.
