Recent surveys of the Champaign County Nursing Home by both the Illinois Department of Public Health and the Champaign-Urbana Public Health District found numerous violations, including improper storage of foods, a lack of hot water, unsanitary kitchen conditions and the failure to meet nutritional requirements for residents.
Investigators also found that foreign objects, including cardboard and plastic, had been found in food served to residents.
Among the findings, obtained Friday by The News-Gazette:
— In January 2012, state inspectors found that a seriously ill patient had lost 10 pounds in a month, an 8 percent loss, and was supposed to receive "health shakes" between meals. But on the inspection day, the patient not only had no dietary order for a health shake, but received only half the lunch serving of chicken and dumplings he was to get.
— During the same inspection, 22 patients were served only half the order of pureed chicken and dumplings they were supposed to get.
— Last December, a dietary manager told the state that she found plastic in the pureed meat served in all four dining rooms. One nurse reported she had found a piece of plastic in a resident's mouth. One large piece of plastic detected in the meat "was the finger part of a glove, from the knuckle to the tip of the finger and that was how she figured out the pieces of plastic were part of a glove," the report said.
— The wife of a resident reported finding "a small piece of cardboard in his pureed food" and said "this is not the first time she has found foreign objects in the food."
— Residents who were supposed to receive 6 ounces of beef stroganoff over 4 ounces of noodles received only 4 ounces of beef stroganoff, nor did they get any of the pears on the menu.
— In an April 11, 2013, inspection, a Champaign-Urbana Public Health District official found that a walk-in cooler was too warm and "was condemned for use until it can maintain food temperatures or 41 degrees or below."
— In the same inspection, a water heater at the nursing home wasn't operating, and water in the dishwashing machine was not reaching 180 degrees.
— In a Dec. 12, 2012, inspection, a can opener blade was soiled with debris build-up, interior oven racks were soiled, dry storage racks were soiled and dusty, and there were no paper towels at a hand sink.
